In patients with PTSD, ambient cortisol levels are lower than normal; this state has been attributed to chronic “adrenal exhaustion” from inhibition of the HPA axis by persistent severe anxiety. 

Two structures in particular have been implicated in the aetiology of PTSD, the amygdala and the hippocampus.
